Here you will find information that will be helpful as you prepare for your upcoming appointment at South Jersey Breastfeeding and Wellness.

 

On our FAQ page we explain what to expect during and after the appointment, questions related to insurance reimbursement, etc.

Please review all of this information and let us know if you have any additional questions or need clarifications. Your appointment is not confirmed until you have sent us confirmation via text or email.

 

Please print and complete the demographic and intake history forms (click the underlined words for the forms)  and bring them with you to your appointment.  If for some reason you are unable to fill out the forms prior to the appointment, please arrive 10 minutes in advance of your scheduled appointment.

On the day of the appointment:

 

•  Your baby should be ready to eat - try to feed him or her about two hours before your scheduled appointment.  But please don't worry if you need to feed again sooner for any reason - we need a happy, hungry (but not frustrated and starving!) baby

•  Please bring anyone you want to be there with you, including your partner, grandma, or other member of your support system

•  Any “tools” you might be using - your pump, nipple shield, etc. We have a Breastfriend pillow in our office in addition to a lot of throw pillows but you are welcome to bring your own nursing pillow if you prefer

• If you are using supplemental feeds, please bring a bottle (or whatever method you are using) of expressed breast milk and/or formula

• Any questions or concerns: It can be helpful to write them down in the time leading up to your appointment and bring them to the consultation.

• Payment- (No out-of-pocket costs for Aetna, Amerihealth & Independence Keystone & Personal Choice BCBS subscribers, some plans may be subject to a co-pay.  Please note we are NOT in network with Horizon NJ or out of state Blue Cross PPO plans)

  • Initial Breastfeeding Consultations 90 minutes (see FAQs for a better understanding of what's included): $175 

  • Follow Up appointments 60 minutes (if needed): $90 

  • Back to Work Consultations 60 minutes: $90

  • Prenatal Consultations 60 minutes: $90 

  • Payment is required at the time of the appointment (other than contracted insurance subscribers) and can be remitted in the form of cash, check, or credit card (cash or check is preferred.) Please see the FAQ link regarding insurance reimbursements for Lactation Services

  • If you are a fee for service client, we can provide you with a coded receipt to submit to your insurance for reimbursement. Please advise your lactation consultant if you would like a receipt.

• Our office is located at 915 Haddon Ave in Collingswood, NJ 08108

 

• There is a 20 car parking lot in addition to ample street parking surrounding our office.
